Statement of Intent For The Lodge Surgery
New contractual requirements came into force on 1st April 2015 requiring that GP practices should make available a statement of intent in relation to the following IT developments:-
- Referral Management
- Electronic Appointment Booking
- On line Booking of repeat prescriptions
- Summary Care Record
- GP2GP transfers
- Patient Access to the detailed information from the medical record.
Dr Blackman, Dr O'Driscoll and Dr Munnelly now offer to our patients all of the requirements under the GP contract relating to IT developments and have worked closely with our Clinical System Suppliers to allow patients to access the detailed information from their medical record.
Patient Access to the detailed information from the medical records is available by application. Patients will need to have an Online Account set up to access this information.
Patients will also need to complete one of the application forms below, whichever is relevant, and return it to the surgery in person with a form of identification. Applications cannot be processed without patient identification. Paper copies of patient information leaflets and forms are also available from reception.
Please note that patients can currently request the Summary Record or Detailed Coded Record. Please do not request the Full Clinical Record service as it is not yet available.

Consent to Proxy Access Online
In order to have proxy access to someone else's records (ie family member or cared for person) both parties must first have their own online account.
Applications take up to 21 days to process.
