Summary Care Record - No Additional Information

What additional information can be added to your Summary Care Record? Essential details about your healthcare can be very difficult to remember when you are unwell so you can choose to add more information to your Summary Care Record. This is then known as an SCR with Additional Information and will help you and those caring for you to know your healthcare details.

The extra information includes details of your:

Long term health conditions such as asthma, diabetes, heart problems or rare medical conditions.

Relevant medical history including clinical procedures that you have had, why need a particular medicine, the care you are currently receiving and clinical advice to support your future care.

Your health care preferences which will make caring for you more in line with your needs, such as special dietary requirements.

Your personal preferences whether they are religious beliefs or legal decisions that you would like to be known.

Details of previous vaccinations such as tetanus and routine childhood jabs.

If this information is not included in your Summary Care Record you may have to remember it and repeat it to healthcare staff treating you.

Specific sensitive information such as fertility treatments, sexually transmitted infections, pregnancy terminations or gender reassignment will not be automatically added unless you specifically ask for any of these to be included.
